Three solumn names are:- Member Name Date Joined Amount Paid I have then filled 5 rows with data beneath those column names. If I then click in cell G 17 (containing the Member Name field header) and click on my Form button (set up in the Quick Access Toolbar) - Form launches corrrectly. Seems to be working OK for me so do not seem to be able to re-produce the problem you are having. If my comments have helped please hit Yes. Thanks.
