There are lots of html "rules", but none of them will relate specifically to the appearance of the "Reader" button in Safari's tool bar. Actually, checking some of my own pages suggests that Safari/Reader is inconsistent in its behaviour - follow any of the "tours" here through, and you'll see what I mean: http://www.chem.utoronto.ca/coursenotes/analsci/instruments.html For example, click on the left-hand image on that page, and watch the "Reader" button appear on the linked page, yet both have images within text, centred within the page! Obviously, Apple have some bugs to work out with this one...

No, I'm saying that the code used in Safari for their Reader functionality is the open source code from Arc90's Readability project.
