It looks as if the disk label has been corrupted somehow. Your best bet
is to init the disk and restore from backup.

If you don't have a recent backup, you have learned a valuable lesson!

You may wish to test the disk drive using some sort of disk exerciser.
If your system is important to you, the last thing you need is flaky
hardware!
