What is the file that iTunes reads to find out where everything is?
Can it be refreshed, looking for all the music in the iTunes
subdirectories and rebuilding?

itunes library file

drag all of your music to the itunes window.

What does the file look like, and were is it located?

I found a backup of itunes/iTunes Library and moved it to her iTunes
library overwriting what she had, then opened iTunes by clicking on
it. It worked.

Well, yes, that would show the state of the catalog the last time it was
used and anything changed. However, it may not show everything that's in
the iTunes Music folder. For that, you can drag and drop the whole
folder full of music onto the iTunes window, and only items not already
in the catalog will be copied into the database. Pretty neat, and been
that way since SoundJam.
