The MATLAB appendix for Balanis' book Antenna Theory: Analysis and Design (3rd Ed.) is not provided online at the author's home page or at the publisher's website.

Aside from buying the book or borrowing it from a library, your options are limited. I very much doubt you'll get much help with backdoor routes to copyrighted material from this newsgroup.
