Word disappears unless it is maximized.
I had the same problem. Here is the actual solution: "Restore the max'ed window using the "square" control in the upper right. This will make the window "disappear". Yes, go to Insert-> field, then select "fill-in". If you save the document as a document template, then every time the document opens, you will be prompted for the fill in.
