I’m using Q2008 HB but have no business items. The tax item “Medical”
was broken into subcategories which included tax item “Drugs
prescription” but also “Medical nondeductible” under which over-the-
counter drugs and equipment fall.

I just made an entry for the later and found that the icon with the
check mark on the red dot appeared at the beginning of the line entry
in the register. This nondeductible category is NOT checked on the
“Category List” as all others are under the Medical category but,
nonetheless, the check mark appears in the registry signifying that
it's a tax item.

The entry isn’t falling through to Schedule A on the tax reports but
that check mark makes me uneasy. It’s NOT a tax item.

Just a quirk of the program I should forget about?

I'm using 2008H&B but can not replicate what you are seeing. I would double
check the category listing for that category. Make sure that the tax-related
box is unchecked and the Tax line item is blank. You can have the
tax-related box checked with a blank Tax line item. This will give you the
icon and not flow through to Schedule A.
