ISA's SMTP functionality has NOTHING to do with spammers, unless you use ISA to block known spammer IPs. The ISA SMTP filter inspects SMTP traffic and takes some steps to ensure that SMTP traffic is not 'malformed', many previous exploits relying on 'malformed requests'. 'out of the box' it does very simple checking, however, as 'a true ENTERPRISE class firewall' the SMTP filtering can be made MUCH more strict.
