In Word 2003 I'd click on"View", "Toolbar" and "Form" to put together a
questionair that would be locked so only the text fields could be filled out.
How do I do this in the newer versions?
Thanks for any help.

The "legacy forms" controls are still available on the Developer tab, which
is not displayed by default. You must check the box for "Show Developer tab
in the Ribbon" at Office Button | Word Options | Popular.
