You can do it, but you need to include special XML code in an add-in; the add-in has to be in the new 2007 format, meaning that it won't work in earlier versions. There's a pretty good book on the subject, "RibbonX, Customizing the Office 2007 Ribbon" by Martin, Puls and Hennig. It does a nice job of explaining things in a way that made sense to me; wish I'd had it a lot sooner. The MS docs for this are (IMO) badly scattered and much harder to follow.
