Surround the horizontal (landscape) page with section breaks, and set
that section's orientation to Landscape. See
http://www.word.mvps.org/FAQs/Formatting/LandscapeSection.htm, which
also discusses the issue of getting the header and footer in the
landscape section to match those in the portrait sections.
