Perhaps permanently, so anyone new can look at it if they wished. It a lot cleaner now than it was before. Either the user code box is too small or the instructions are wrong. One can still answer 5years+12months as well as 6years+0months. And the calendar year is now 2010 and in just eight months will be 2011 (hint). You might want to consider a simple one character alt text for that circle thing. Imagine what would happen if the image was not available. Why not use ● (bullet) as the alternate. Better yet, lose the image and just use it K.

In comp.lang.javascript message <0r2cr5teakgfvrun3fbecetjg57icembe0(a)4ax. com>, Fri, 2 Apr 2010 16:21:33, Geoff <geoff(a)invalid.invalid> posted: >http://www.micro-active.com/a-wm2/a-q-student-wm2.htm Even if you intend only to print that out or otherwise send it to the clients, it would IMHO be well to include, perhaps separated from the Questionnaire by a Rule, such reference items as a link to other parts of the site, a date, a name, a means of offering suggestions. If you put year, month. and day in that order, you can adjust the length of the month before readers get to that; and it would be prettier if all were of the same width (I'm looking with FF 3.0.19). But, IMHO, people can enter DoB faster as YYYY-MM-DD than with three drop-downs, and validation is easy enough onBlur or otherwise. Given DoB and date, do you need to ask Age? Select Months should use Jan-Dec or 1-12, not 0-11. To me, Age of diagnosis means "How long diagnosed?", Others may take it as meaning "Age at diagnosis?". It might be useful to ask *which" spoken language is preferred, and whether English will be satisfactory. If the form is filled in on the Web, can a Google Translate version be used? Could you tell?
