Volume indicator (without hitting the volume keys) = glitch. Volume slider =
normal operation (on home button double-click).

Volume indicator, I think - double click makes it appear.

Along with play/pause buttons, right? That's the normal operation (on double
click).

I've had the volume indicator (the one that appears when you click the
up/down volume buttons) appear randomly too - just a glitch.

If you mean the iTunes-player control, you get that if you dbl-click the
"home" btn when it's locked. It's a feature: it means you can pause, rewind
&c without having to unlock it (eg when it's in a dock).
