Unlike some, I too have seen this from time to time, though rarely. An example: copying material from a browser window (using command-C), then attempting to paste into a different application (using Command-V, or via the Edit menu) and finding a previously copied chunk is pasted instead. I don't know why it happens, but I have noticed that sometimes it's reproducable, and that when it happens my Command-C keyboard stroke doesn't flash the 'Edit' on the menubar. So (in my experience) it's the key combo that for some reason is not being acknowledged. Copying the source again, but using the Edit menu itself, has always done the business for me.
