It's for long distance excise taxes billed from Feb 28, 2003 through Aug 1,2006. The IRS web site in September said that Form 8913 was going to be the magic incantation for the rebate. It did not exist when I looked. The default rebate probably will be a line item in the regular 1040x forms. However, it could all have changed since the last time I looked. My itemization is $40.
